Option 1: Manual Application at Civil Registration Offices

  1. Prepare the Required Documents:

    • Copy of the lost birth certificate (if available).
    • Notification of birth or baptism card (if applicable).
    • Copies of the applicant’s or parents' National IDs.
    • Sworn affidavit stating the loss of the birth certificate.
    • Police abstract for the lost certificate.
  2. Visit the Nearest Civil Registration Office:

    • Head to your local Civil Registration Office or Huduma Centre.
  3. Fill Out the Replacement Form:

    • Provide details such as:
      • Full name as it appeared on the original birth certificate.
