Documents Needed for ID Replacement

  1. Police Abstract

  2. Sworn Affidavit

    • Draft and swear an affidavit confirming the loss or damage of your ID.

  3. Copy of Your Lost ID

    • If available, provide a copy of your lost or damaged ID card.

  4. Birth Certificate

    • A certified copy of your birth certificate.

  5. Parents’ ID Copies
